Who is Eligible?

 

Realty ONE Group network members with a registered:

  • Limited Liability Company (LLC)
  • S-Corporation, or a
  • Sole Proprietor with an Employer ID Number (EIN) from the IRS

 are eligible to enroll in the Standard Major Medical Plan powered by TRIAD.

  • If you are a Sole Proprietor without a EIN, please click here to learn more from the IRS on how to obtain one (it's free and takes about 15 minutes).
  • There are many good reasons to have an EIN as a sole Proprietor such as setting up a business bank account, issuing or receiving 1099-NEC/1099-MISC forms under a business name, etc. Please consult with your tax advisor.
  • Sole Proprietors without a EIN are eligible for the 1099 Sales Professionals program.

Business with 25 or more enrolled have flexibility in:

  • Faster underwriting without the need for health questionnaires. 
  • The number of plans being offered to their employees,
  • The PPO networks can be customized, and
  • The plan designs themselves can be customized.

How it works

 

Enrollment is available year-round. When you enroll your business, your insurance coverage will be effective on the first of the month of your choosing. 

While these plans cover pre-existing conditions, they are underwritten.  During the application process, businesses with fewer then 25 enrolled employees will have each employee complete a health questionnaire.  Business with 25 or greater will only need to provide a census with detailed employee information.

  • PLEASE NOTE: The rates shown in the summary may be adjusted lower after underwriting.

Plans through TRIAD provide you with tools to navigate the healthcare system including:

  • Full PPO Network provided by CIGNA.  Including Hospitals. Search Cigna's PPO (Instructions)
  • Option to select a Physician Only PPO Network with direct contracts to facilities for premium savings of about 12%.  Schedule time with Arcwood to learn more.
  • The plan Pharmacy Benefits Manager, Disclosed Rx, is skilled at reducing specialty medications to $0 for the member.

Start your STANDARD Major Medical quote by completing the form below.

When you hit submit, you will receive an email with two instructions. First, as the plan administrator to add your employees names and email address; Second to complete your own personal health questionnaire (PHQ) for enrollment. 

When this is completed, your employees will receive an email to complete their PHQ. 

Once the applications are complete, underwriting will take about a week.

Dental Insurance

MetLife offers a National Network, check for your dentist here:
  • Please select the PDP Plus Dental Network. If your dentist is not contracted with MetLife, you may nominate them, there are also out-of-network benefits for non-contracted dentists too.
  • Available to Businesses of ONE person, or multiple employees.
  • Two options to choose from, Base and Premium, your choice if you or your employees contribute to the plan(s).
  • Single Premium's begin at $35 per month; Families from $70 to $150 per month.
  • Rates are based on your zip code. Rates shown below are examples for Arizona.

Details:

  • Preventive Services covered at 100% 
  • Basic Services covered at 80% 
  • Major Services covered at 50% 
  • Annual maximum benefits $1,500 or $2,500 per member.

Vision Insurance

Metlife offers a National network of Vison Providers using the VSP Vision Network.
  • Choose from a large network of ophthalmologists, optometrists and opticians, from private practices to retailers like Costco®Optical, Walmart, Sam’s Club and Visionworks.
  • Single Premium $10 per month; Families from $19 to $27 per month.

Details:

  • Plan offers exams, lenses or contact lenses, and Frames once every 12 months.
  • Savings on glasses and sunglasses: Get up to 20% savings on additional pairs of prescription glasses and non-prescription sunglasses, including lens enhancements. At times, other promotional offers may also be available.
  • Potential savings averaging 15% off the regular price or 5% off a promotional offer for laser surgery including PRK, LASIK and Custom LASIK. This offer is only available at MetLife participating locations.

Life Insurance

Basic Life Insurance for you and your employees.

  • $50,000 basic life for each eligible employee in your company, each employee names their own beneficiary.
  • Higher amounts are also available for owners and leadership if desired.
  • Rates are based on the total amount selected and the number of employees; however, a company of 10 for the basic $50,000 of coverage would cost ~$70 per month as reference.
Supplemental coverage is available for employees.
  • Employees can apply for up to $400,000, Spouse and Child Life Insurance also available.
  • Guaranteed Issue when individuals are first eligible of $150,000 for employees, $20,000 for spouse/domestic partner. 
  • Future enrollments, or amounts over the guaranteed amount will require underwriting.

  • Additional Features included: Grief Counseling, Accelerated Benefits, and conversion to continue the coverage after termination.

Disability Insurance

Disability Income protects your, and your employees, most valuable asset; earned income.

 

Weekly Disability Benefits.

  • Protects up to 60% of income to $5,000 per month in benefits.  Insuring $8,333 per month of earned income.
  • Begins after 3 months to co-inside with the Short-term disability benefits.

Additional Benefits

Accident Insurance are benefits that help cover the costs such as those not covered by your medical plan.

You’ll have a choice of two plans (called the “Low Plan” and the “High Plan”) that provide payments in addition to any other insurance payments you may receive.
