MetLife offers a National Network, check for your dentist here:
- Please select the PDP Plus Dental Network. If your dentist is not contracted with MetLife, you may nominate them, there are also out-of-network benefits for non-contracted dentists too.
- Available for independent contractors and our employers with employees.
- Two options to choose from, Base and Premium, your choice if you or your employees contribute to the plan(s).
- Single Premium's begin at $35 per month; Families from $70 to $150 per month.

- Annual maximum benefits $1,500 or $2,500 per member.

Vision Insurance
Metlife offers a National network of Vison Providers using the VSP Vision Network.
- Choose from a large network of ophthalmologists, optometrists and opticians, from private practices to retailers like Costco®Optical, Walmart, Sam’s Club and Visionworks.
- Single Premium $10 per month; Families from $19 to $27 per month.
Details:
- Plan offers exams, lenses or contact lenses, and Frames once every 12 months.
- Savings on glasses and sunglasses: Get up to 20% savings on additional pairs of prescription glasses and non-prescription sunglasses, including lens enhancements. At times, other promotional offers may also be available.
- Potential savings averaging 15% off the regular price or 5% off a promotional offer for laser surgery including PRK, LASIK and Custom LASIK. This offer is only available at MetLife participating locations.
Life Insurance
Basic Life Insurance for you and your employees.
- $50,000 basic life for each eligible employee in your company, each employee names their own beneficiary.
- Higher amounts are also available for owners and leadership if desired.
- Rates are based on the total amount selected and the number of employees; however, a company of 10 for the basic $50,000 of coverage would cost ~$70 per month as reference.
Supplemental coverage is available for employees.
- Employees can apply for up to $400,000, Spouse and Child Life Insurance also available.
- Guaranteed Issue when individuals are first eligible of $150,000 for employees, $20,000 for spouse/domestic partner.
- Future enrollments, or amounts over the guaranteed amount will require underwriting.

Disability Insurance

- Protects up to 60% of income to $5,000 per month in benefits. Insuring $8,333 per month of earned income.
- Begins after 3 months to co-inside with the Short-term disability benefits.
