When you click "Let's Go!", you will receive an email with two instructions. First, as the plan administrator to add your employees names (if nesessary) and email address; Second to complete your own personal health questionnaire (PHQ) for enrollment. 

If you have employees added, once you complete the plan administrator form, your employees will receive an email to complete their PHQ. 

Once the applications are complete, underwriting will take about a week.

Who is Eligible?

 

ASID members with a registered:

  • Limited Liability Company (LLC)
  • C or S-Corporation, or are a
  • Sole Proprietor with a Employer ID Number (EIN) from the IRS

 are eligible to enroll in the TRIAD Major Medical Plan with the CIGNA PPO.

  • If you are a Sole Proprietor without  EIN, please click here to learn more from the IRS on how to obtain one (it's free and takes about 15 minutes).
  • There are many good reasons to have an EIN as a sole Proprietor such as setting up a business bank account, issuing or receiving 1099-NEC/1099-MISC forms under a business name, etc. Please consult with your tax advisor.
  • Sole Proprietors without a EIN are eligible for the Individuals program.

 

How it works

 

Enrollment is available year-round. When you enroll your firm, your insurance coverage will be effective on the first of the month of your choosing; typically it will be the first month following 30 days to allow for education and enrollment to your employees. 

While these plans cover pre-existing conditions, they are underwritten.  During the application process, employers with fewer then 25 enrolled employees will have each employee complete a health questionnaire.  Employers with 25 or greater will only need to provide a census with detailed employee information.

  • PLEASE NOTE: The rates shown in the summary may be adjusted lower after underwriting.
